Social prescribing enables GPs, memory services and local agencies to refer people to a link worker for non-clinical support for their wellbeing, to discuss what matters most to them. Link workers, familiar with local options, can direct them to social stimulation opportunities, such as arts, to empower them to take greater control of their own health.
